FRONT Pet Name/Tag Text (15 characters)

Add your pet’s call name or some custom text for the front of your tag. We recommend keeping the text to a few lines, which would include Pet phone number. (2 lines)

BACK Pet Name/Tag Text (15 Characters)

Add your pet’s name, phone number or other information. We recommend keeping the text to a few lines. We will work to accommodate the text provided; however, if we run into issues, we will provide additional options.

Pet Phone Number (15 Characters)

We want to make sure we get your pet’s contact information correctly. Please enter your preferred phone number for folks to use should they find your pet and need to contact you.

Custom/Special Requests

Enter any special requests or personalization you would like to see. We’ll work with you to ensure you get the pet tag you desire.

KPETS Donation: A donation to the KPETS organization will be made for all Pet Tags personalized for KPET Therapy Pups.
